GENERAL CONDITIONS:

• Billing & Cancellation: Membership fee is auto-charged monthly on the original subscription date. To cancel a membership, please contact Leisure Day Spa at least 30 days before the next billing date.

• Monthly Services Usage: Services included must be used within the corresponding monthly cycle. Unused services DO NOT roll over to the following month.

• Appointments & No-Shows: Appointments must be canceled or rescheduled at least 24 hours in advance. Missed or late-canceled appointments will be considered USED for the billing cycle and will not be rescheduled.

• Discount Limitations: Discounts provided through the membership cannot be combined with other existing promotions or prepaid packages.

• Birthday Gift: Complimentary birthday gift must be claimed within your birthday month only. Not transferable.

• Initial Consultation: Client agrees to undergo a professional consultation to determine the best treatment. 

• Locked-In Rate: Your $87/month rate is guaranteed for life as long as your membership remains active without interruption.

• Modifications: Leisure Day Spa reserves the right to modify benefits or terms of the membership with a 30-day notice to active members.

• Non-transferable: Membership is personal and cannot be transferred to another person.
